This is a clinical picture in which the patient has a positive history of acute or chronic stress conditions, mood disorders, and anxiety disorders caused by personal or work situations.

PSYCHOGENIC ALOPECIA

A growing body of scientific evidence indicates that psychophysical stress conditions can alter both hair growth in the laboratory animal and hair growth in humans. Historically, the influence of stress has always been considered in the context of both alopecia areata (AA), telogen effluvium (TE), and androgenetic alopecia (AG). Several clinical observations suggest the emergence of a stress-related diagnostic picture with distinctive features, which can be termed Psychogenic Alopecia (PA).

Exposure of the body to psychophysical stressinduces activation of the hypothalamic-pituitary-corticosurrenal axis with release of CRH, ACTH, POMC and cortisol. R1 receptors for CRH are also found to be expressed at the level of keratinocytes, fibroblasts, and sebocytes, where they are involved in increased sebum production.Psychophysical stress also appears to activate the release of certain mediators, such as NGF and substance P, whose effects at the level of the pilosebaceous unit include perifollicular neurogenic inflammation, apoptosis of keratinocytes, and induction of catagen. Together, these biological phenomena would contribute to the cynic features observed in psychogenic alopecia.

Diagnosis:

• Hyperseborrhea and seborrheic dermatitis;
• Trichodynia referred most often to the vertex;
• Perifollicular inflammation;
• Diffuse thinning to the entire scalp with hairline sparing;
• Sometimes hyperhidrosis;
• Positive history of acute or chronic stress conditions, mood disorders, anxiety disorders, personality disorders, psychotic traits.
