There are multiple ways of administering pharmacological active ingredients: of these, the oral route is undoubtedly the one of greatest use, and therefore of greatest importance. When active ingredients are taken orally, they can be sublingually absorbed, orosoluble or enterically absorbed products: the rate of absorption increases gradually going from enterically absorbed to orosoluble to sublingually absorbed products.[1]

Of particular interest is the treatment of biotin deficiencies: it is, in fact, involved in multiple biological processes, playing a role in DNA transcription and the inflammatory response. In addition, being an indispensable coenzyme for the proper functioning of all enzymes of the carboxylase class, biotin deficiencies are often associated with the development of seborrheic dermatitis, as defects in metabolism and fatty acid production can lead to excessive sebum production and thus seborrheic dermatitis as a second cause. In other cases, this deficiency can reduce immune responsiveness by promoting the proliferation of Malassezia fungi associated with the condition.[2] Biotin deficiency can also lead to hair loss, development of skin rash, and weakening of the immune system.[3

Low levels of this vitamin can have different causes in adults and children: in children the problem is usually congenital and due to a deficiency in the enzyme biotinidase; in fact, all newborns are tested for this genetic disorder. Biotinidase is required to break the covalent bond that forms between biotin and proteins that use it as a coenzyme: in the absence of this enzyme, biotin that is normally introduced through the diet cannot be absorbed from food and is therefore lost. However, dietary supplementation of this vitamin is sufficient in children to solve the problem, while in adults it often is not.

In addition to biotinidase deficiency, other causes of biotin deficiency may be:

  • Gut dysbiosis: this vitamin can be sequestered and absorbed by certain bacteria in the intestinal flora, which, if present in excessive amounts, then result in failure of absorption by the host.
  • Alcoholism: alcohol abuse reduces the intestinal absorption capacity of many micronutrients, including biotin.
  • Use of anticonvulsants (barbiturates), antibiotics, or the intake of raw egg white: all of these three facts reduce biotin absorption because they either make the action of biotinidase more difficult (the drugs) or because they contain compounds that bind to biotin with such strong bonds that they cannot be broken even with the help of that enzyme (the proteins in raw egg white).[4]

The effects of biotin deficiency are not to be underestimated, even and especially at the psychological level on the patient’s well-being: aesthetic and easily visible problems can be largely presented; therefore, intervening with adequate supplementation is necessary, especially with supplementation that can bypass intestinal malabsorption problems.

Clearly, a method of administration that completely avoids intestinal absorption, which would bring little benefit to the patient anyway, is of considerable importance in adults to counteract acquired biotin deficiency.

Intravenous biotin supplementation can be opted for, but this is psychologically worse tolerated by the patient than standard os therapy, resulting in lower therapeutic adherence. Alternatively, orosoluble or sublingual biotin formulations may be considered.

Absorption, in both orosoluble/orodisperse and sublingual formulations, occurs maximally through the oral mucosa, minimizing intestinal absorption, which presents particular problems for patients with acquired biotin deficiencies.[5]

Between orosoluble/orodisperse and sublingual formulations, moreover, it is more advantageous to use the sublingual formulation, which is found to have more favorable absorption kinetics.

This is especially true for the use of sublingually absorbed spray preparations, as the small drops of the spray have a much higher concentration of active ingredient than the whole solution, evaporating more rapidly even on the way from the dispenser to the application substrate. This higher concentration allows a significant increase in diffusion toward the blood through the barriers of the sublingual capillaries, resulting in rapid bioavailability of the active ingredient and the possibility of therapeutic responses even with lower dosages. Consequently, this promotes better patient compliance and satisfaction.[1]
