Indroduction/Premise

It is now well known in medicine and specifically in Trichology that in women, deficiencies from estrogen deficiency can lead to the occurrence of Ludwig-type Female Alopecia “FAGA” (Estrone Carential Alopecia) or to a Telogen Effluvium. Let us therefore briefly describe these two conditions. Estrone Carential Alopecia, often considered a variant of Female Androgenetic Alopecia, today affects about 35% of women. As the name implies, the causative factor is a deficiency, either in the production or in the action, of estrone by the follicle locally. Probably caused by reduced activity of aromatase and/or of the 3 alpha reductase. Aromatase, 3 alpha reductase, and the 17-betasteroid reductases convert androstenedione and testosterone to estrone. Estrone maintains matrix mitoses, maintains the durability of the anagen and activates stem cells at the beginning of anagen itself.

L’Estrone Carential Alopecia manifests as a condition of hypotrichia. Patients manifest a loss of “mass” and thinning of hair. The pattern is centrifugal, usually at like Ludwig, also affects non-androgen-dependent parts of the scalp such as the nape of the neck. There is no miniaturization with reduction in follicular depth but rather in thickness and thin hair remains long and deep.

Telogen Effluvium is the most frequent cause of trichological examination. Hair loss diffusely affects the entire head, including the sides and back. When effluvium is significant enough to cause thinning the resulting alopecia is homogeneous over the entire scalp. We usually distinguish acute, subacute, and chronic Telogen Effluvium. Chronic Telogen Effluvium, certainly the most severe, is a disorder that almost exclusively affects the female sex, has a multifactorial genesis, and is characterized by profuse hair loss (more than 100 hairs per day). It may be associated with a chronic inflammatory state.

The cause or causes are not intuitive and often nuanced: persistent psychological problems, inadequate diets, drug administration, chronic autoimmune diseases such as SLE, ulcerative colitis, etc… Among the very frequent and insidious causes of Telogen Effluvium are chronic anemias of all kinds and autoimmune thyroiditis in hypothyroidism. Telogen Effluvium frequently is accompanied by Tricodynia (annoying and continuous pain referred to the base of the hair or scalp).
