SYMPTOMS
Among the disorders concerning the gastroesophageal tract, the most common are digestive difficulties and gastroesophageal reflux.
Digestive difficulties are grouped under the generic term dyspepsia (from the Greek dys-pepsia, meaning “poor digestion”), while the term gastroesophageal reflux refers to the retrograde passage of stomach contents through the lower esophageal sphincter into the esophagus.
Heartburn, also called heartburn or acidity of the stomach, is a common symptom. In gastroesophageal reflux, the burning sensation indicated by the patient radiates from the center of the abdomen (epigastric region) toward the jugular. In dyspepsia, on the other hand, the symptoms are mainly localized to the central area of the upper abdomen (epigastrium).
Heartburn may occur in association with:
– dull pain in the upper abdomen (up to and including the jaws)
– nausea and vomiting
– belching or bloating
– feeling of full stomach after meals
– hypersalivation
– decreased appetite and weight loss
– insomnia.
Gastro-oesophageal reflux disease (GERD) is referred to as a “disease,” but it is actually a syndromic condition that can manifest itself through various forms, divided into esophageal and extraesophageal syndromes
CAUSES
When heartburn is an occasional problem, the patient does not struggle to identify its root causes:
-meals that are too large
-foods that are difficult to digest or have an effect on the esophagus:
-Fatty foods tend to stay in the stomach longer as they are slower to digest
-Foods such as chocolate and mint may have a relaxing effect on the lower esophageal sphincter, favoring the rise of stomach contents
-Spicy foods, onions and tomato products, very hot or acidic drinks may exert an irritating effect
-taking meals before bedtime
-taking food too fast
-taking alcohol and cigarette smoking
-taking medications (e.g., NSAIDs).
Other causes may be:
-ineffective esophageal clearance capacity:
-gravity
-peristalsis (due to impaired gastroesophageal sphincter function)
-inherent bicarbonate salivation and production (acid salivary pH)
-Pregnancy-induced physical and hormonal changes
-Hyatal hernia
-Insufficiency of the lower esophageal sphincter (LES)
-Transient SLE rebleeds
-Delayed gastric emptying
-Abdominal obesity
-Emotional factors: anxiety and stress
-Helicobacter pylori infection
TREATMENT
The first approach in the treatment of gastroesophageal problems is to act on the symptomatology, thus counteracting the burning, and then assisting gastrointestinal function and motility.
It is essential to eliminate or at least reduce risk factors such as the intake of NSAIDs, obesity, smoking alcohol and rebalance the diet by avoiding excesses and potentially irritating foods.
GASTROWIKEN
Gastrowiken, a dietary supplement based on Licorice, Banana Glyceric Extract, Lentisk and Sodium Hyaluronate, is useful in cases of heartburn by controlling gastric acidity and restoring normal gastrointestinal function and motility.
PROPERTIES OF CONSTITUENTS
Banana plays an active role in controlling gastric acidity and regularizing digestive functions.
Alginates are assimilated to soluble fibers. The viscous colloidal solutions formed by alginates in contact with the mucous membrane of the esophagus have a mucoadhesive effect that improves symptomatology in erosive and nonerosive diseases also in contact with gastric acids and in the presence of calcium forms a protective layer that reduces the upflow into the esophagus of stomach contents.
Calcium carbonate in contact with gastric acids generates carbonic acid that releases carbon dioxide gas. The carbon dioxide allows the viscous colloidal solution formed by alginate to float on the stomach contents promoting antireflux action.
Sodium hyaluronate also protects the stomach and esophagus from acid in a mechanical way. Chemically, hyaluronic acid is classified as a glycosaminoglycan being formed by repeating long sequences of two simple sugars, glycuronic acid and N-acetylglucosamine. Its high solubility in an aqueous environment is important for ensuring tissue hydration while protecting it from excessive tension and stress, and its high affinity for other hyaluronic acid molecules and other components of the extracellular matrix allows the formation of a dense and intricate high-molecular-weight network.
The use of licorice extract as an anti-ulcerative is widely known; in fact, it is used in gastric and duodenal ulcers and as an adjunct in the treatment of spasmodic pain in chronic gastritis. These benefits are related to the presence of anti-inflammatory saponins. In addition, licorice prolongs the life of stomach surface cells, with a demonstrated anti-pepsin effect. Clinical studies on deglycyrrhizinated licorice have shown effects in the treatment of gastrointestinal ulcers not dissimilar to those obtained with untreated licorice, suggesting the presence of other active ingredients in the protection of gastrointestinal tract function.
Lentisk acts on digestive system function and intestinal transit regularity. The resin secreted from the stem of Pistacia Lentiscus, is effective against Helicobacter pylori infections, in particular, terpene components of the essential oil in the resin inhibit the growth of H. pylori.
